Fistula Treatment


Fistula in ano is a common anorectal problem through out the world .It is defined as a narrow track lined by granulated tissue communicating with the anal cnanl and perianal skin . It has got usually a single opening in the anal canal or in the rectum but may have one or more opening in  peri anal skin.
Changing trends in the treatment is due to the fact that none of the surgical method proved satisfactory.the recurrence complications are too many .The main problems faced in the surgical procedure are
 1)Extensive mutilation of ano rectal and ischio rectal area .
2)Prolonged hospitalization.
3)High rate of recurrence.
4)Sphincteric incontinence.
Complications of surgery like sphincteric incontinence , 
Stenosis,proctitis and fissure in ano make the life of the patient worse.
Fistula can be a challenge to the most genious surgeon.further the success rate does not exceed more than 50% .
Patient has to stay for long period in the hospital for surgical dressing once or twice daily .dressing itself is very painful and scary for the patient .
Many attempts have been made to minimize the period of dressings by either primary closure of wound or by skin grafting but results are unpredictable.
Thus the surgical treatment of fistula in ano still presents a number of practical problems which stand as a challenge to the treating surgeon.
